Please use this form if you are considering Peaceful Home Euthanasia for your pet. Fill it out as best as you can to assist both yourself and Dr. Susan to either prepare for your appointment or to assist Dr. Susan. For "Quality of Life" Consultations and Peaceful Home Euthanasia, Dr. Susan travels to the following areas: Surrey, White Rock, Langely (Centeral & West Langley only), Vancouver, Burnaby (South of Hwy 1), New Westmister, North Delta & Richmond. As above, I am closed currently , please check in December 2017.
We apologize that Dr. Susan does not routinely provide ongoing medical services to cases in these areas as she cannot always provide the repeat, or follow up care that your pet may need, given her home location.
